We know you’re busy, so our normal business hours include lunchtime, evening, and Saturday morning appointments. We’re open for appointments and phone consultation during the following times:
MON - THUR | 8:00am - 5:30pm
FRIDAY | 8:00am - 4:00pm
SATURDAY | 9:00am - 11:30am
If you leave a message outside of our normal business hours, we’ll get back to you within one business day.
Please note there is a $45 fee for missed appointments.
In the event of an emergency, call 911. If you need to speak to a doctor after business hours, you can page the on-call physician at 502.241.6567.
Sometimes insurance providers will consider prescribed medicine or treatments “non-preferred,” despite your doctor’s recommendation. If your insurance provider has designated a recommended treatment “non-preferred,” we will need to submit a pre-authorization form to your insurance. Should a pre-authorization form be necessary, there is a $25 fee at the time of service. For more information, see our [[full pre-authorization policy.]]
Please bring a list of your medicines to your appointment, so we can update our records and refill your prescriptions. Should you have refill requests between appointments, contact your pharmacy, and they will send us a refill request. If you need to make any changes to your prescription or your request is denied, give us a call at (502) 241-6567.
Our office accepts most forms of insurance. Please bring your insurance card with you to each visit. If applicable, be prepared to pay your copay or deductible.
We will bill your primary insurance company after your treatment, but we cannot give you estimates or promise that a treatment will be covered. Your policy is between you and your insurer, and all policies are different. Contact your insurer for estimates.
