Dr. Tony Karem

Dr. Karem received his undergraduate degree from Eastern Kentucky University, and completed both medical school and his residency at the University of Louisville. He is interested in internal medicine and pediatrics because he likes getting to know whole families, helping care for each family member’s medical concerns at all stages of life. A doctor who cares for an entire family has a more complete understanding of each member’s needs. Dr. Karem knows all about caring for a big family—he’s a dad to six children, and he loves taking his kids to football and basketball games. On the weekends, you’ll find him out in his vegetable garden or fishing at the lake.


Dr. Ilana Kayrouz

Dr. Kayrouz was born and raised in Louisville, KY. Her grandfather was a doctor, and her father worked at the hospital, so she imagined a future in healthcare from a young age. She completed medical school and residency at the University of Louisville, while raising two children (who were two and six years old when she started)—a challenge that made her grateful for the support of friends and family. She especially enjoys working at IMPA because of the “small town feel” of Crestwood, the flexibility and individual attention that a private practice makes possible, and the big-picture perspective of working with adults and children of all ages. When she’s not working, she loves to travel and compete in races, including marathons and triathlons. Her husband owns a restaurant in Louisville.


Dr. Spencer Idstein

Dr. Idstein grew up in Louisville, KY, attending Saint Xavier High School. He graduated from the University of Dayton in Ohio prior to earning his medical degree from the University of Louisville School of Medicine. He completed his combined residency in internal medicine and pediatrics at the University of Alabama at Birmingham in Birmingham, AL. Practicing both internal medicine and pediatrics he loves the opportunity to see patients of all ages, often caring for multiple members of the same family. He appreciates being able to follow his patients throughout their lifetime. Outside of work you will find him spending time with his wife and family. He also enjoys fishing, kayaking and playing tennis.


Dr. Nicole Strecker

Dr. Strecker was born in Louisville, KY. She attended Sacred Heart and then pursued gymnastics in college at the University of Denver. She completed medical school at the University of Kentucky and returned home to the University of Louisville for her residency training. She became interested in the specialty of internal medicine and pediatrics because she loved the idea of getting to know the whole family and providing medical care for each of their unique needs at every age. She was also excited to be able to provide continuity to adolescents as they transitioned from pediatrics to adulthood. Dr. Strecker fell in love with the community of Crestwood while working at IMPA as a resident and is excited to join the medical staff. When she isn’t working, she enjoys spending time with her family, going to any UL sporting event, and going to the lake.
